On this page you can download and print the forms you’ll need to complete prior to your initial consultation with board-certified plastic surgeon Dr. Sean Hill. While you’re welcome to wait to fill them out, completing these forms ahead of time saves you time and allows you to focus exclusively on your goals and treatment plan at your appointment.
